DNA damage and cellular genomic instability are fundamental processes associated with the development of cancer and degenerative diseases, resulting from failures in genetic repair mechanisms. The aim of this study was to analyze the relationship between DNA repair and cellular genomic stability, considering its impact on tumor progression and therapeutic response. A quantitative explanatory approach with a non-experimental design was applied, based on secondary data from international organizations such as the World Health Organization, the International Agency for Research on Cancer, and the Ecuadorian Ministry of Public Health, covering the period 2021–2023. Statistical methods included Pearson correlation, logistic regression, Kaplan-Meier survival analysis, and principal component analysis. The main results showed a strong positive correlation between mutations in repair genes and tumor progression (r = 0.84), higher cancer risk in carriers of BRCA1, BRCA2, and ATM mutations, and improved survival rates in patients treated with PARP inhibitors. Oxidative stress and replication errors were identified as the main contributors to DNA damage. It is concluded that genomic stability depends on the efficiency of DNA repair mechanisms and their interaction with environmental and metabolic factors, highlighting the importance of targeted therapies.
